INDIVIDUAL MEMBER CONSENT TO USE OF ELECTRONIC TRANSMISSIONS

In accordance with Sections 20, 21 and 5079 of the California Corporations Code and Section 5.06 of the Amended and Restated Bylaws of the California Partnership to End Domestic Violence, a California nonprofit public benefit corporation (the “Corporation”), the undersigned hereby agrees to, and acknowledges, the following:

1.The Corporation may send or provide meeting notices, annual reports, ballots and all other materials (“Records”) to me by “electronic transmission” and may use the e-mail address listed in Item 3 below for that purpose. For purposes of this Consent, “electronic transmission” means a communication delivered by (a) E-mail, (b) Posting on an electronic message board or network, together with a separate notice to the recipient of the posting or (c) Other means of electronic communication.

2.The Corporation may rely on communications sent by me to the Corporation, or a third party service provider retained by the Corporation, by electronic transmission for any purpose, including action by written consent or by written ballot. I hereby certify that the Corporation may reasonably conclude that I am the author of communications so sent.

3. I understand that I have the right to have all records provided or made available on paper or in non-electronic form, but only if requested by me in writing.Any such request may be mailed to the Corporation at 1107 9th Street, Suite 910, Sacramento, CA 95814, attention Membership, or may be sent via email to the email address set forth in Item 4 below. Requested records shall be provided within a reasonable period of time after receipt of a request therefor.

4. This Consent will remain in full force and effect until I revoke it.To modify or revoke this consent, I acknowledge that I must notify the Corporation of such modification or revocation by e-mail to the following e-mail address membership@cpedv.org.
